We now have an automated system for your convenience. Sometimes you're so busy that you need answers right away. And, some of you work hours other than 8-5, and it's problematic to try to contact customer service during those hours.

We installed a new, automated system to help you get your answers when you need them, 24 hours a day. Here's an overview to help you navigate the system more easily.

  1. Dial 800-228-5088 or 614-508-2255.
  2. Enter the social security number of the member under whom the coverage is listed. If, for example, both you and your spouse are State employees, and the husband is listed as the employee (member) for coverage, you would enter your husband's social security number.
  3. Enter the number associated with the member's Union:
    • 1 OCSEA/AFSCME
    • 2 DISTRICT 1199/SEIU
    • 3 OSTA
    • 4 FOP/OLC
    • 5 SCOPE/OEA
    • 7 CWA
  4. Listen to your menu options.

Main Menu

  • For dental plan help, press 1.
  • For vision plan help, press 2.
  • For life insurance plan help, press 3.
  • For Working Solutions help, press 4.
  • For disability gap plan help, press 5.
  • For other questions, press 6.

Once you enter your choice from the main menu, you can retrieve specific information regarding that benefit.

DENTAL or VISION

You can obtain the telephone numbers for contacting the various plans, check your eligibility or obtain forms.

LIFE INSURANCE

Basic life insurance (press 1) You can find out your coverage amount, obtain a beneficiary form, hear Prudential's phone number or learn how to file a claim.

Supplemental life insurance (press 2) You can obtain your monthly premium amount, hear Prudential's phone number, request forms or get claim or termination information.

WORKING SOLUTIONS

You can get Working Solutions' contact information, hear a description of their services and verify that you are eligible.

 OTHER QUESTIONS

You can request the most recent newsletter or an information packet on the benefits available to you. You'll want to verify your address within the system, and if there's a discrepancy, you will need to update your address with your Payroll/Personnel officer.
