We've just posted a new OPEN ENROLLMENT
section for a limited time only (5/17 to 6/4, which just happens to be
the Trust open enrollment dates...)

Benefit Information
Information regarding Trust benefits
can be found in the "Benefit Plans"
section. There were only a few changes to the plans and your options for
the '99-'00 plan year; you can find these in the "New"
section.
Provider Information
Reviewing the most current provider
information may help you decide whether to keep or change your current
plan.
Look in the "Links"
section for a quick connection to those insurance companies with websites.
VSP, MetLife and Prudential HealthCare are all there, as is Cole (for
open enrollment only). These plans have networks, and these networks
may have changed since you chose your plan.
Making a Decision
Our feature article of the month,
found in the "New" section, deals with
choosing the best plans for you and your family based on what you expect
your health needs will be in the upcoming plan year.
Open Enrollment Information
Quick bits of information
such as deadlines, what benefit changes are possible and where to send
the forms are listed in the special Open Enrollment section. Another area
to check for details is the "FAQ" section.
This section, which also has a special Open Enrollment posting, answers
the most frequently asked questions of Trust customer service through
our 800-line or website.
Forms
Download your forms - in the "Forms"
section, you'll find the:
- Open Enrollment form,
- Supplemental Life Insurance
form and
- Beneficiary form.
You can print and submit the
Open Enrollment form to your Payroll/Personnel officer or Supplemental
Life Insurance or Beneficiary forms to Prudential. The deadline for returning
all forms is June 4, 1999.
F0RMS & DEADLINES
JUNE 4, 1999 is the deadline for all Trust benefits.
When using the Open Enrollment form to add or change your dental or vision coverage, please make
sure the form is filled out completely, then turn it in to your Payroll/Personnel officer by June 4, 1999. This coverage will be
effective July 1, 1999.
When using the Supplemental
Life Insurance form to add or change your supplemental coverage
or that of your dependents, please make sure the form is filled
out completely, then mail it to Prudential by June 4, 1999. Depending
on when your form is processed, your coverage will be effective
either July 1 or August 1, 1999 - check your paystub
for the new supplemental life insurance deduction.
Again, it is very important
that your Beneficiary form is
up-to-date. Don't let your coverage be delayed in reaching your
family when they need the coverage most.
