1999 Trust-Sponsored Open Enrollment

It's that time again...from now until June 4, you have the opportunity to elect dental, vision and life insurance coverage for you and your family for the upcoming plan year, 7/1/99, through 6/30/00. If you already have coverage, now is the time to make changes to suit your anticipated needs. To make changes or enroll for the first time, submit the appropriate forms no later than June 4.

Why your kit may not
arrive by May 24

The main reason why many members don't receive Trust communication in a timely fashion is because the State doesn't have your current address on file (so neither do we). If your mailing address has changed over the past year and you didn't notify your Payroll/Personnel officer, you may not receive your kit in time to meet the enrollment deadlines. Always make sure your address is current with your Payroll/Personnel officer so you receive your benefits news and updates.

Also, most of the materials we mail to you are sent via the US Post Office's third class, or "Bulk," rate. This helps us better control costs, but sometimes takes longer to arrive at your home; however, we plan accordingly and allow plenty of time for the kits to arrive (they were mailed to you May 4).

The Trust recently mailed your open enrollment kit to your home address. Let us know if you haven't received the kit by May 24, and we'll make sure you receive one so that you have enough time to consider your options. If you don't have your kit by May 24,

The kit has information only about enrollment options available to you. It also includes highlights on the benefits under which you are covered automatically.

For all Trust members, regardless of service length

Supplemental Life Insurance

You can provide additional income protection for your family by purchasing supplemental life insurance. If you already have supplemental life insurance through the Trust, you may increase your coverage by one times your basic annual earnings without a medical exam or other proof of insurability. Additional coverage is available, but requires evidence of insurability. You may also cover your dependent spouse and children with supplemental life insurance equal to $10,000 for your spouse and/or $5,000 for each eligible child.

Working Solutions Service

Working Solutions offers expertise and resources to help with problems or issues you and your dependents may face. Under Working Solutions, a "dependent" includes a relative, friend or neighbor for whom you have responsibilities that affect your ability to work. A dependent may be any age and does not need to be a blood relative. You may obtain services for your own special needs as well.

Working Solutions offers services for two categories of dependents:

  1. Adult/Elder Dependents
  2. Special Needs Dependents

Call Working Solutions anytime for information and referrals. Just call 800-358-8515 and tell them you are covered by Union Benefits Trust.

You can receive up to four free packets during this enrollment period.
