Trust Customer Service
We're Here For You
The Trust has the unique ability to focus solely on your coverage needs because we exist to serve
your benefits needs. And Trust customer service has the same ability to concentrate solely on answering your questions about Trust-sponsored coverage.
If you have questions, please contact us for the answers.
You have several options as to how you'd like to reach us - we have a staffed Customer Service
line, available from 8 a.m. - 5 p.m., Monday through Friday, an automated phone system (IVR) and a thorough web site: http://www.benefitstrust.org.
- If you're calling Trust customer service - Contact the Trust at 614-508-2255 if you're
in Columbus, or 800-228-5088 if you live outside Columbus. Enter your social security number and number that corresponds with your union to
speed your connection.
- If you're using the IVR system - Call 614-508-2255 if you're in Columbus, or 800-228-5088.
Enter the information requested, and listen for the benefit about which you have questions. You can check your eligibility, get contact information
for the plans, or request information be faxed or mailed to you. Between April 24 and May 12, a special section will be available with critical
enrollment information.

Don't Forget
All members are also covered by Working Solutions Service.
If you have at least one year of State service, you are also covered by Basic Life.
This coverage provides one times your Basic Annual Earnings in life insurance to your beneficiary if you die while covered. Confirmation of new beneficiary
designations or any changes you make to your beneficiary (ies) will be mailed to you by Prudential. All members are also covered by Working
Solutions Service.
This information and referral service will be enhanced July 1, 2000, to offer even more specific
and thorough help for your dependents: dependents with special needs, including children; adult/elder dependents; or, children - birth through

Attn: DMO Members
If you are a member of the Prudential Healthcare DMO®, please remember that your dental coverage
will be maintained. The DMO will end June 30, 2000, and as of July 1, 2000, you'll be covered automatically by the Preferred Choice Plan (unless
you elect Quality Dental during open enrollment). Remember to contact MetLife for network provider information,
so that you'll receive higher levels of coverage by using network providers for dental services.
