Reinstating UBT benefits for eligible dependents terminated on November 30 as a result of the state’s dependent audit

If members’ eligible dependents were terminated on November 30, 2009 as a result of the state’s dependent audit conducted by SECOVA, UBT will reinstate dental and vision coverage, with the required documentation through June 30, 2010. Members will receive a confirmation letter stating the dates the UBT benefits have been restored.

IMPORTANT: If you do not turn in the required documents to the state of Ohio during the 2010 enrollment period, your dependents will not have any coverage for the new benefit period, which begins July 1, 2010.

How to reinstate dental and/or vision coverage for eligible dependents:

  • Review the documents required for eligible dependents here. Members must include Name and Employee ID at the top of each document.
  • Submit the required documentation to UBT via fax 614.508.0025; email or mail to:

  • Contact UBT’s Customer Service to verify dependent’s coverage is reinstated before making any appointments. 1.800.228.5088 or 614.508.2255.

Reminder: Members must submit dependent documentation to the state of Ohio during the 2010 open enrollment to continue UBT benefits after June 30, 2010.
