This Way To 0pen Enrollment

Between April 23 and May 11, you have the opportunity to elect dental, vision and life insurance coverage for you and your family for the upcoming plan year, July 1, 2001, through June 30, 2002. If you already have coverage, you may want to review the changes we've made to some of the plans.

You are not required to complete a form to take advantage of the improvements to the vision and dental plans. To make changes or enroll for the first time, submit the appropriate forms no later than May 11. Supplemental life and beneficiary forms go to Prudential, but enrollment forms go to your Payroll/Personnel officer. All of the materials you need were mailed recently to your home in your enrollment highlights booklet. If you don't receive your booklet by April 30, download the booklet now.

The enrollment highlights has limited information about the Trust enrollment options available to you. It also contains some highlights on the benefits for which you are covered automatically. For the real detail, please see the plan booklets that you received in late March/early April.

Note: As the open enrollment period has ended, the documents only valid for open enrollment are no longer online.
